NOTES/COMMENTS:
Greetings Veterans, Fellow Leaders and Delegates:

As you are all aware the scheduled trip to Juno Beach is quickly approaching. As President of the Manitoba Metis Federation and Minister in charge of Veterans Affairs for the Metis National Council, I am pleased to send the following information to the Manitoba Delegation attending Juno Beach from November 8th to 14th, 2009.

Marlin Travel will be providing the MMF will all finalized travel itinerary for the Juno Beach Trip including hotel information within the next week. However, please find attached preliminary information that details departure date and time, as well as a preliminary agenda of the activities that will be undertaken through the week.

Please note that the MMF has secured a physician to accompany our Delegation to Juno Beach. Dr. Clement Lang will be the attending physician for our trip and for ease of access requires a healthcare form be completed for each Delegate in advance of our trip.

It is recommended that due to H1N1 and to ensure that everyone’s medical information is provided to Dr. Lange for review prior to departure that the attached medical form be completed.

Also, it is of utmost importance that all Delegates see their current physician for a check-up prior to travel. Along with the medical form please submit a note from your current physician stating that you are in good health to travel.

Please have the attached healthcare form along with your current physician’s note submitted to the attention of Shannon Allard -Chartrand at home office or via fax (204-947-1816) as soon as possible.

Please note that overseeing the coordination of the Juno Beach Trip is Shannon Allard-Chartrand. Mrs. Chartrand can be reached at the MMF Home Office at 586-8474 Extension 365 should you have any questions or require any further information.

Lastly, a liability waiver form will also be sent to each Delegate under separate cover next week for completion prior to departure.
